All log entries are due by the last date of the pay period. This does not replace the timesheet available in Banner, which must be submitted by the last day of each pay period. Pay will not be submitted retroactively.

By typing my name and Banner ID, I verify that all information on this form is accurate. Falsification of this official school record is cause for disciplinary action.
